There has been an increase in survey tags related to incontinence and bladder assessment. How can this be avoided?

A: Per the October 2015 State Operations Manual, Synopsis of regulation (F315),
the urinary incontinence requirement has three aspects. The first aspect requires that a resident does not have a catheter inserted unless the resident’s clinical condition clearly demonstrates that it was necessary. The second aspect requires the facility provides appropriate treatment and services to prevent urinary tract infections; and the third is that the facility attempts to assist the resident to restore as much normal bladder function as possible. It is important that facilities clearly documents the medical need and care plans for catheter use, including prevention of UTIs. Equally important is documented evidence of completion of Voiding Diaries and Bladder Assessments to determine the type of incontinence, as well as the completion of a detailed care plan based upon that assessment.
